Employees can adopt AI faster than an organization can see or govern its use. They may use personal accounts, unapproved services, or separate departmental tools because these solve an immediate problem. Useful experimentation can then become embedded in business work without clear responsibility for data handling, access, output review, or continuity.

The diagnostic raises this concern when unsanctioned use is reported as widespread and no published tool policy provides clear guidance. The label is deliberately strong, but it is not proof that a breach has occurred. It points to a gap between actual working practices and the organization's ability to understand and manage them.

Establish an interim policy within 30 days. Make it usable: explain which tools and uses are approved, what information may be entered, who remains responsible for checking outputs, and where employees can obtain a timely decision. Pair it with an anonymous discovery exercise to understand what people use and why. Provide a fast route for evaluating useful tools and an approved alternative where existing options are inadequate.

Treat that discovery as operational intelligence. Unofficial use may reveal unmet needs, unclear guidance, or an approval process that does not work. Address those causes while containing specific exposures. A policy becomes effective when employees can follow it without making ordinary work impractical.

Assessment signals reflect self-reported responses and invite investigation; they do not predict project outcomes. Suggested action deadlines should be adapted to your organization.
