When failure modes appear together

Several signals can describe different parts of the same management problem. Their combination can change the sequence of action, but it does not establish causation. Use the following pairings to decide what to investigate first.
Pilot Purgatory and Measurement Vacuum
A fragmented portfolio with weak measurement is difficult to govern. There may be promising work, but leaders lack a common basis for deciding what deserves further investment. Continuing everything preserves the problem; canceling everything can discard useful capabilities.
Review strategic fit and evidence of value together. Ask each owner to establish the best available baseline, identify the next test, and accept a dated funding decision. Close work that cannot justify continuation, and give credible candidates a bounded opportunity to prove their case. Missing measurement does not make a pilot worthless or politically easy to cancel. It makes a fair, consistent review more necessary.
Frozen Middle and Change Capacity Deficit
When managers are reluctant and teams lack the capacity to learn, a major redesign places more demand on the people least equipped to absorb it. Another mandate or motivational campaign will not resolve the conflict between delivery obligations and transformation work.
Address workload, incentives, and role-specific learning before a broad rollout. Involve managers in designing a small, supported trial so that learning and practical change can progress together. Expand when teams can perform the new work reliably. The combination argues for careful sequencing, rather than assuming that technical readiness is sufficient.
Data Foundation Risk and Operating Model Stagnation
Here, both information and the process using it require attention. A data program without a defined workflow can become unbounded, while redesign based on unreliable information can fail as soon as it encounters live conditions.
Choose a single workflow and address both together. Let the intended outcome determine which data defects matter first; let the actual data constraints inform the redesign. Assign business and data owners a shared delivery objective. Demonstrate a repeatable result before extending the approach to other workflows.
Shadow IT Crisis and Governance Bottleneck
This combination should prompt an investigation into whether slow approvals are contributing to unofficial use. Employees may be bypassing a process that cannot meet operational needs, although unclear policies, unsuitable approved tools, and other factors may also be involved.
Improve the approval route and provide workable alternatives while addressing specific exposures. Make the conditions for permitted use clear and give teams a predictable decision time. Faster approvals alone will not resolve every issue, but stricter enforcement without an accessible route to legitimate use can leave the underlying demand untouched.
Technology-First Trap and Strategic Misalignment
When spending follows technology novelty and leadership has not agreed its priorities, purchases can begin determining the strategy by default. Each commitment creates another reason to continue, even if the original business rationale remains unclear.
Put a temporary hold on discretionary new AI purchases while the executive team names its priorities. Assess the existing portfolio against those choices and fund the people, process, and data work required by the retained initiatives. Keep the review time-bound and preserve necessary support for live services. Resume new commitments when leadership can explain the business outcome, the owner, and the complete path to delivery.
Turning a signal into an executive decision
Use your diagnostic report to choose the conversations that need evidence. For each material signal, identify what would confirm or challenge the interpretation, who owns the relevant decision, and what change can be tested within a defined period. Review the combinations before launching separate workstreams that compete for the same people.
The value of naming a failure mode lies in the intervention it makes possible. A useful response ends with a clearer decision, an accountable owner, and observable progress in how the business operates.
